Fatality Investigation: Oversized Scaffolding Platform Destabilizes and Telescopic Forklift Overturns

On a June afternoon in central Kentucky, a siding installer died in a horrific accident involving a forklift truck and a platform upon which he and a co-worker were standing. It is hoped that the investigation of the events surrounding this fatality will prevent similar incidents.
